I hereby consent and authorize General Motors LLC, OnStar LLC and General Motors of Canada Company, and their respective affiliates and agencies, (collectively, the “GM Parties”) to use my full name, title, story (as provided above), audio recordings, and personal information (collectively “Materials”), or any material based upon or derived therefrom, without payment or any other compensation. I understand that these Materials may be used worldwide for advertising, marketing and any other lawful purposes as the GM Parties shall deem proper, and in any and all media, whether now known or hereafter existing or devised, in perpetuity, and for any other use that the GM Parties, and those acting pursuant to their authority, deem appropriate. This consent and authorization is irrevocable and continuous, as well as transferable and assignable by the GM Parties in their sole discretion. The statements or any portion thereof made by me (as reflected in my story) for use in the Materials mentioned above are in my opinion true and accurate and reflect my true and honest opinion of and experience with the GM Parties.

I agree that any and all advertising materials (the “Advertising Materials”) created by or for the GM Parties that incorporate, in whole or in part, the Materials shall remain the sole and exclusive property of the GM Parties and that the GM Parties shall retain all intellectual property rights, including, without limitation, copyright, that exist in the Advertising Materials. To the extent that I may have any moral rights in the Advertising Materials, I hereby waive all such moral rights in favour of the GM Parties (and anyone authorized by the GM Parties to use the Advertising Materials).

I agree that the Materials can be modified or edited in any way without my consent or the consent of any party or person. Further, I agree that I shall have no right of approval, no claim to additional compensation or benefit, and no other claims arising, directly or indirectly, out of the use of the Materials. I understand a member from the GM Parties may contact me in regards to the Materials I have submitted.

I further agree to release, defend, and hold harmless the GM Parties and their respective advertising agencies from all liability and claims arising from or related to the use of the Materials in any manner and media. I warrant that the Materials I have submitted are free of any abuse of copyright law and do not violate another third party’s intellectual property or other rights, including but not limited to invasion of privacy, right of publicity and trademark infringement.

Furthermore, I confirm that I have read the attached statement. I affirm that this statement reflects my honest opinions, beliefs or experience with the product featured in the statement. I also confirm that I was at the time I gave the statement, and I continue to be, an owner/lessee or user of the Chevrolet, Buick, GMC, and/or Cadillac product featured in the statement.
